Vendor Sponsorship Registration Two Day Symposium November 6th and 7th, 2019

Registration

• Addition Vendor Staff – $35.00
  Only one vendor representative is allowed with the vendor registration. If vendor has addition staff wishing to attend to support the vendor table, we encourage this practice for a small additional price. This price will not include lunch.
• Gold – $600.00
  *Vendors @ the Gold level will have table to display information/products for the entire day.
  *2 dedicated hours of exposure to clinicians.
  * Gold level includes full contact list for all registrants. Vendor hours are Nov 6th 5:15 pm-6 pm. Nov 7th 7:30 am - 8:00am, 11:15 am - 1:15 pm and 2:15 pm - 2:45 pm
  
  ** Includes full registration including lunch for 1 individual representing the company. Additional company representatives must register & pay the normal individual conference fee.
• Platium – $750.00
  *Vendors @ the Platinum level will have table to display information/products for the entire day. *2 dedicated hours of exposure to clinicians.* * Platinum level Sponsors will have first choice of event table location in the event space. This is first come first served so please do not delay in registering.
  * At the end of the conference, we will give away Platinum sponsored prizes. Platinum sponsors will be displayed during prize give away on projector screen.
  * Platinum level includes full contact list for all registrants.

  ** This level provides Platinum level booth space and benefits. It is reserved for those vendors having also sponsored a speaker.
